Vendor Risk Assessment

In order to comply with campus policy, we need to complete a VRA (Vendor Risk Assessment) for your software purchase or online service.  This UC-wide policy is in place to provide integrity and confidence that any data which may be created, stored, or transmitted is done in a way that is compliant with privacy and security to protect each of us individually, as well as an institution.

The VRA process can take a few days to months, depending on many factors including the type of data and where it is stored. We recognize this lead time can be a source of understandable frustration so please plan accordingly.

To start the VRA process, please answer the following questions to help us gather the needed information

If you are not the PI or manager for the account/fund source specified, please provide the name of the PI or approver who can authorize the use of these funds. If CRP funds cover the entire purchase amount, please enter your name in this field.
If you need this item by a specific date, please specify that here. Note that computer orders will take at around 2-3 weeks once the order has been placed with the vendor, longer if it is back-ordered or there are other shipping delays. Purchases over $5,000 are considered capital items and require additional accounting processing time.
If needed, please provide any special instructions or requirements for this purchase. E.g., indicate if you would like to order a specific color or any special peripherals, cables, etc. as part of this order. E.g., indicate if this order should be split across multiple accounts.
